Summary
404 Girl is the girl who pops up whenever someone tries to access a 4chan thread that has fallen off page 15, and thus been autodeleted. She can also pop up if you try to access any dead 4chan link.
Origins
404 Girl is known as Yotsuba in Japan. Yotsuba is Japanese for "four-leaf", and thus she fits 4chan well. She has green hair tied into four pig tails and is used as a 4chan mascot along with that moot girl thing.
Yotsuba comes from the Japanese manga Yotsuba&! (from the guy who also writes Azumanga Daioh). In this comic, she is an adopted and naive and energetic five year old girl who doesn't understand how anything works.
She started being referred to as 404 Girl when one possibly troll anon asked /b/ for rule 34 on the 404 girl. This post occurred in April of 2007 and led to numerous facepalm jpegs. This rule 34 was instantly considered taboo, and thus many trolls have since repeated the request.

Naturally, as a response to this meme, much Yotsuba Rule 34 has since surfaced.
Current status
404 Girl is still a 4chan mascot and can still be seen on all 404'd pages on the website.
