Summary
"You're doing it wrong" is a catchphrase that has become associated with image macros, most notably fails. When on an image, the phrase is usually modified to (activity): you're doing it wrong.

Origins
The first appearance of this phrase was in 2004, when the domain name YoureDoingItWrong.com was registered. It was later replaced by Doing It Wrong, which generates "doing it wrong" pictures and allows users to submit their own.
Related gifs and images started showing up on 4chan and YTMND a short time later.
Current status
This meme is still going strong, and shows up frequently.
